Day Habilitation (Site Based)
Site based Day Habilitation services are person-centered and assist individuals to learn, retain or improve an array of skills in their chosen goals to live more independently. The site based Day Habilitation services are provided in a classroom environment as well as out in the community. This service provides an individual the ability to learn socialization, adaptive, and communication skills in an educational and fun manner, through activities of their choosing.
Day Habilitation WOW (Without Walls)
Day Habilitation WOW services are provided primarily in the community. Individuals that receive this service attain their self-chosen goals through the skills that are learned/strengthened while integrating in the community. Participants work together to do research, participate in local activities, volunteering opportunities, discussing current events and exploring new things.
Individuals who have an I/DD (Intellectual and/or Developmental Disability) diagnosis, are self-toileting, able to access program vans while self-ambulating, Medicaid Waiver enrolled, and have made an informed choice to participate.
