HOME AND COMMUNITY BASED SERVICES

We help adults age 21 years of age and over choose, obtain, and keep employment. We can help with managing symptoms related to a behavioral health conditions and strengthen wellness and self-care practices to get you integrated into your community.

Gateway is approved by the New York State Office of Mental Health to provide Home and Community Based Services (HCBS) to eligible Medicaid beneficiaries referred to us by managed care organizations in our community. Our continuum of services supports New York State’s goal of transitioning Behavioral Health services to a person centered, community-based system that supports recovery.

There are three components to our Home and Community Based Services (HCBS):

  • Employment
  • Habilitation
  • Psychosocial Rehabilitation.

Employment

Gateway assists individuals across Ulster and Dutchess Counties to find and maintain employment. We work with adults in Ulster and Dutchess Counties who have a psychiatric diagnosis, intellectual and developmental disabilities, economic challenges, and other barriers to employment. We cover the full spectrum of employment services:

Habilitation

  • These services are designed to assist participants in retaining and improving the skills needed to reside successfully in home and community-based settings.

Psychosocial Rehabilitation

Individuals with personal and/or environmental barriers associated with their behavioral health conditions may need some extra assistance in achieving a goal of being fully integrated in their community. Services include:

    • Restoring interpersonal, self-management and basic living skills
    • Relapse prevention
    • Managing symptoms related to behavioral health conditions
    • Strengthening wellness and self-care practices
    • Obtaining and maintaining roles in integrated community settings

Eligibility

HCBS are for individuals who are enrolled in Health and Recovery Plans (HARPS) via a Medicaid managed care program. A Health Home Care manager will typically conduct an annual HCBS eligibility assessment. When eligibility is confirmed, the care manager will also develop a Person-Centered Plan of Care, and then assist in working with the individual in selecting an HCBS provider (like Gateway).
